Hello there,
I'd think the "maximum username changes" option would only count actually successful changes, and not rejected ones as well. We now had a user that we rejected two times (and the maximum being two), and he cannot put in another request, although both of his previous requests (because the names violated our ToS) were denied and thus no username change has actually taken place.
So I'd suggest to add an option "Maximum username change requests" to go with "Username change interval", with "Maximum username changes" only counting the actual changes.
